Inclusion/Exclusion Criteria

Inclusion Criteria

  • Aged between 2 and 23 months

  • Documented fever of ≥ 38.0°c (tympanic, axillary, or rectal)

  • EVENDOL score ≥ 4 (see below for more information about the EVENDOL score)

  • EVENDOL score completed ≤ 60 minutes of arrival to ED


All four inclusion criteria must be met for a child to be eligible

  • Paracetamol less than 4 hours or ibuprofen less than 6 hours before time of screening

  • Already received maximum 24 hour dose of paracetamol or ibuprofen

  • Known paracetamol or ibuprofen sensitivity

  • Requiring immeidate treatment for possible sepsis

  • Clinically unstable as determined by treating clinician

  • Active gastrointestinal bleeding or gastric ulcers

  • Chronic renal failure, liver failure, or significant cardiac disease

  • Known coagulopathy (eg. Haemophilia, ITP etc)

  • Gross global developmental delay

  • Malignancy

  • Parent/guardian not present

  • Previously enrolled in PIKI study

    None of the above exclusion criteria must be present in order for a child to eligible

Study Flow

  1. Screen for eligibility

  2. Obtain verbal consent

  3. Randomise

  4. Study medicine 1 & 2 administration

  5. After 90 minutes - first assessment

  6. Study medicine 3 & 4 administration - if needed

  7. After 4 hours - final assessment

  8. Parent/guardian satisfaction score
